what's wrong with me?

Required Pre-departure Documents:

IMPORTANT
Please note that we have not received the following required predeparture documents:
1. A completed and signed Rotary Insurance Certificate form ( located in the Scholar Handbook )
2. A completed and signed Rotary Medical Certificate form ( located in the Scholar Handbook )
3. A copy of the identification page of your passport
4. A copy of your visa
5. A completed Travel Option Form ( Option 1, 2, or 3 )
6. Electronic Funds Transfer Bank Information Form (located in the Scholar Handbook)

Please note we cannot release your scholarship funds or your airline ticket until we receive the above documents. You may fax copies to my attention or you may email copies of the documents to me.
